About Parkview Medical

Our Story

In 2002, volunteers began providing wellness services under a nearby oak tree within the Oakland Community of downtown Haines City, FL.

In February of 2009, Parkview Outreach Community Center, a non-profit community organization, opened it's doors for the first time to the local community of Polk County. Offering free medical care services, health screenings and education, food and clothing distribution, and other skills-trainings for the surrounding local residents.

Now operating as Parkview Medical Clinic, we still find ourselves nestled right into the heart of downtown Haines City and Oakland Community; eager to serve the underserved and uninsured population of Polk County residents.

Fun fact: Our building is now yellow!

Our Mission

At Parkview, our mission is to educate and empower the uninsured, and underserved population of Polk County to take full responsibility for their healthcare; and to utilize community organizations, and resources available to them.

Our Services

Parkview Medical Clinic offers compassionate medical care through adult primary care services, psychiatric & mental health services with our in house PMHNP, well women visits. and an in-house pharmacy.

Parkview also partners with other local organizations to offer specialty referral services through programs to elibible patients. These can include cardiology, gastroenterology, radiology services, labs, pain management, and many others!

What Services Does Parkview Offer?

Parkview Medical Clinic offers general medical care, specializing in hypertensive and diabetic diagnosis', mental health services with our in-house Psychiatric-Mental Health Nurse Practitioner, and well women visits.

How Can I Become a Patient? (Eligibility Requirements)

Parkview offers free medical care services to patients who qualify under our eligibility.

To become a patient you must:

Provide proof of Polk County, FL residency (lease or rental agreement, roomate/ familial statment, or updated ID with address in Polk County)
Provide a copy of your ID & Social Security Card (if-applicable)
Provide 1-months proof of income (bi-weekly: 2 paystubs, weekly: 4 paystubs) if actively working

Does Parkview Medical Clinic See Children?

Parkview does NOT see children, eligible patients must be over 18 years of age to be seen in the clinic.

Parkview does however, offer two to three events during the summer months providing FREE back to school physicals for school age children (5-18) in person at the clinic. Keep an eye out for flyers in the community of posted dates for the upcoming events or stop by the clinic to ask for a flyer.

Does Parkview Allow Students and Providers for Clinical Rotations/Hours?

Yes! Parkview loves partnering with the local medical community by hosting medical students and/or providers needing to complete clinical hours, or through other programs like FRAME.

We often have NP, RN, M.A, and LPN students in the clinic completing their roation hours. Email us for more information.
